If you would like to schedule an appointment with us, please call our office at (773) 275-0110. To make your first visit easier
please bring the following to your first appointment:

- Completed registration form
       If your medical history includes any of the following, please let us know when you schedule your appointment:
               - Joint replacement
               - Artificial heart valve
               - History of infective endocarditis
               - Congenital heart condition
- If you have dental insurance, please bring the completed insurance form.
- List of current medications

If there are recent radiographs (x-rays) of your mouth, please make arrangements to have them sent here.

Although we are not a member of any managed care plan, we do accept most insurance plans. But that's only half the story;
coverage also depends on whether or not your insurance will pay for the services provided by us. This depends on what type of
insurance plan you have.
